How to Join the Plan

Please scan the QR code to register or come into the practice and collect a leaflet.

You may withdraw from the Castle Street Dental Plan by giving three months’ notice in writing to CODEplan. If you leave the plan within 12 months of receiving discounted dental treatment the amount of the discount received will be payable to the dental surgeon immediately’.

If you fail to book in or attend your routine appointment on time, the practice will be unable to back date or refund for the missed appointment. If you change address or bank details, please notify CODEplan and the practice.
